Varicoceles

Varicoceles is a common condition affecting men worldwide. It often causes discomfort and can potentially impact fertility. A varicocele develops when veins in the scrotum expand as blood accumulates in them rather than flowing out efficiently.

Varicoceles can lead to swelling in the scrotum and may impair testicular development, leading to low sperm count or even infertility. Early diagnosis and personalised treatment can greatly improve the condition, helping men maintain a healthy reproductive life.

At Dr. Kamaraj Hospital, we understand that male reproductive health can be a sensitive topic to discuss. Our specialised diagnostic and treatment services are designed to support men in managing their reproductive health. Our team of experts in men’s sexual health uses innovative diagnostic tools and treatment methods to deliver personalised care.

Varicocele Symptoms

Symptoms and Diagnosis of Varicoceles

In many cases, varicoceles may not present any symptoms or cause pain and, as a result, may go unnoticed. However, if it does cause pain, the discomfort can range from dull to sharp and may increase with physical activity or with time. Lying down may help ease discomfort by reducing strain on the veins. Other symptoms of varicoceles include:
  • Swelling in the scrotum or testicles
  • A lump in the affected testicle
  • Visibly enlarged veins in the scrotum, which can look like a bag of worms
  • Differently sized testicles
  • Shrinking of the testicles
These symptoms of varicoceles are often diagnosed by professionals through various tests, including:
  • Physical Exam: The doctor may perform a physical examination of the testicles to see if they are swollen. Since a varicocele might not be noticeable while lying down, the examination may include inspecting the testicles while standing up.
  • Valsalva Maneuver: This method is often used to diagnose smaller varicoceles. It entails the doctor examining the scrotum as the patient stands up, takes a deep breath, and bears down while holding their breath.
  • Blood Test: A blood test may be performed to determine the hormone levels in the blood, specifically follicle-stimulating hormone (FSH) and testosterone.
  • Ultrasound: In some cases, a scrotal ultrasound may be required to assess the spermatic veins and to obtain a thorough diagnosis of the condition.

Causes of Varicoceles

  • A varicocele typically occurs on the left side of the scrotum. While the specific cause of varicoceles is uncertain, they are commonly associated with blood flow issues in the spermatic cord. The spermatic cord carries blood to and from the testicles and supports up.
  • The spermatic cord contains arteries, nerves, and veins. If the valves in the veins do not close properly and interfere with blood flow, blood backs up, resulting in a varicocele.
  • Varicoceles can affect men of any age and prevent normal testicular growth.

Types of Varicoceles

When medical professionals diagnose varicoceles, they may classify them on a grading scale of 0-3 based on the size of the lumps:

Grade 0

This is the smallest type of varicocele and can only be seen with an ultrasound.

Grade 1

This occurs when the varicocele is not visible but can be diagnosed using the Valsalva manoeuvre.

Grade 2

This occurs when the varicocele can be felt without the Valsalva procedure but not visible.

Grade 3

This is the largest type of varicocele and can be seen easily. It may also change the shape of the scrotum.

Varicoceles Treatment

Treatment Offered for Varicoceles

The varicoceles treatment depends on the grade and severity of each case. For low-grade varicoceles that do not cause symptoms or impact fertility, treatment may not be required. However, based on the individual case, doctors may recommend:
  • Lifestyle changes: Avoiding strenuous physical activities or any activity that might cause discomfort.
  • Icing: Applying ice or cold packs on the affected area of the scrotum can help ease pain and discomfort.

Other varicoceles treatment options, such as surgery, may be necessary if the varicocele causes pain, shrinking testicles, or fertility issues.

FAQ

What is the treatment process for varicoceles?

Low-grade varicoceles might not require any treatment or may be treated with lifestyle changes, icing, or over-the-counter medication. Surgery may be suggested for severe cases.

Healthcare specialists may recommend various diagnostic tests for varicoceles, such as an ultrasound, a physical examination, or a Valsalva manoeuvre.

The cost of treating varicoceles varies based on the severity of the condition and the chosen treatment options.

Depending on the severity of the case, medications such as nonsteroidal anti-inflammatory drugs (NSAIDs), such as naproxen and ibuprofen, may help relieve pain in the scrotum or testicles.
